Works fine on Loin, you have to install on a PowerPC Mac and install the UB patch. Then copy over to the computer using Lion. Works great over 400 fps.
Doesn't work in 10.8
Some problems in 10.7
I'm putting the finishing touches on the all-new RoaringApps, with support for Mavericks & iOS 7.
Try it out now and help me squash some bugs.

Runs fine on Lion and Snow Leopard just install on PPC Mac.
at one point Quake 3 ran flawlessly using OpenGL on Mac OS X, unknown if Lion breaks that ability, unknown if it runs on 10.6, I know it runs on 10.4.

You need to install using PPC Mac then copy to the lion installation.
PROBABLY doesnt work since it is PPC only. I am not aware of an Intel-Patch being out there?!
I know it works with 10.3 and 10.4, which are Intel/PPC Universal Operating systems, maybe it does work on Intel then?
Can anyone confirm the latest Mac OS release you have this game running on?

Can anyone confirm if it works on 10.5 or 10.6? I can only confirm 10.4 support.

I'm aware there is an open-source version of quake3 for all platforms, ioquake3.org.
Does anyone know if the ioquake3 is compatible with official retail quake 3 clients in multiplayer / lan gaming?
— If ioquake3 and retail quake 3 are compatible this would solve the Quake 3 Arena compatibility question (if ioquake3 runs on Lion).

ioquake3 is a Universal Binary, should be kept updated to work on Lion at some point.
—- ioquake3 at this point is a separate app until compatibility with retail quake 3 is confirmed or denied.
The question is whether or not ioquake3 and original retail quake3 are compatible in multiplayer gaming?
If they are, then ioquake3 should superseed original retail Quake 3 Arena.
if not, then there is a point to specifically define the latest Mac OS compatibility fo this app.
